A safeguarding system tuned to catch everything catches far too much. A school receiving hundreds of flagged incidents a day does not have a detection problem, it has a triage problem, and alert fatigue eventually produces the exact failure the system was built to prevent. Under the Online Safety Act that failure is now a regulatory event as well as a human one, which makes the audit trail matter as much as the detection.
